PER DIEM Licensed Speech Therapists are being sought for the RUTLAND COUNTY and ADDISON COUNTY AREAS performing home visits for a Medicare certified home health care agency.

You, as a Speech Therapist  ST, will be responsible for conducting initial assessments, developing and implementing a plan of care, communicating with the physician and other Bayada and PNSI health care team members, obtaining physician orders as needed, reevaluating client's progress and revise/updating the plan of care, maintaining current clinical notes and records, and participating in team conferences.
